Adult Autism Waiver (AAW)

TEM Care provides the following services under the AAW:

The Adult Autism Waiver is one of two programs in Pennsylvania specifically designed to help adults with autism spectrum disorder participate in their communities in the way that they want to, based on their identified needs.

Behavioral Specialist Services

(BSS)

BSS provides specialized interventions that assist to increase adaptive behaviors to replace/modify challenging behaviors of a disruptive/destructive nature that prevent/interfere with the participant’s inclusion in home/family/community life.

Community Support

(CS)

Community Support assists participants in acquiring, retaining, and improving communication, socialization, self-direction, self-help, and other adaptive skills necessary to reside in the community.

Transportation-Public

(ISP)

Public transportation services are provided to or purchased for an individual to promote self-determination and enable an individual to gain access to employment, services, and activities specified in the Individual Support Plan. 

Respite

Respite provides planned or emergency short-term relief to a participant’s unpaid caregiver when the caregiver is temporarily unavailable to provide supports due to non-routine circumstance.

Assistive Technology

An item, piece of equipment, or product system, that is necessary to increase, maintain, or improve a participant’s communication, self-help, self-direction, and adaptive capabilities.

Supported Employment

Supported Employment services are individualized services, to provide assistance to participants who need ongoing support to maintain a job in a self-employment or competitive employment arrangement that meets a participant’s goals.

Consolidated, Person/Family Directed Support, Community Living Waivers

TEM Care provides the following services under the Consolidated, P/FDS, and Community Living Waiver:

The Consolidated, P/FDS, and Community Living Waivers are designed to help individuals with an intellectual disability, or developmental disability to live more independently in their homes and communities and to provide a variety of services that promote community living, including self- directed service models and traditional, agency-based service models. TEM Care Behavioral Health provides the following services under these waivers:

Behavior Support

Behavior Support service (BS) is provided to participants with behaviors, including destructive, disruptive and disturbing behaviors, which make it difficult for them to be active in their community or to live at home. 

Vocational Assessment

Vocational Assessment evaluates the participant’s preferences in order to develop a Vocational Profile which is an set of actions, tasks or skill development that will position the participant to become competitively employed.

In Home and Community Habilitation

Home and Community Habilitation services are provided to assist consumers in the acquisition, retention, or improvement of skills related to living and working in the community.

Job Finding

Job Finding is an individualized service that provides assistance to the participant in developing integrated employment that fits the participant’s and the employer’s needs. 

Supported Employment

Supported Employment services are individualized services, to provide assistance to participants who need ongoing support to maintain a job in a self-employment or competitive employment arrangement that meets a participant’s goals.

Community Tours - From Meaningful Days to Meaningful Lives

 

Fun Filled Learning Experiences

 

Community Tours is a signature program offered by TEM Care to support our participants in maximizing the benefits of their communities. In our exciting and informative ventures, we employ person-centered, strength-based, and transitional strategies that promote access to and facilitate participation in events and activities that allow for more meaningful daily experiences.

Our program is designed to increase self-advocacy, social development, and transition skills through various civic, recreational, and non-work-related activities. Participants learn what their communities offer and how to access the resources needed to enjoy meaningful lives one day at a time. Some of the things participants enjoy during “Community Tours” outings include: Fire Station Tours, Community Meetings, Meetup Groups, Cooking Classes, Trips to City Hall, Parenting Classes, Food Pantries, Community Gardens and Local Farms, Yoga Classes.

TEM Care Community Tours program promotes Choice, Balance, and Independence by familiarizing participants with the resources available in their communities during individual and group excursions. We believe everyone should have knowledge of, as well as access to, people, places and things that promote meaningful experiences. Our program aims to increase resourcefulness and initiative through exposure to a versatile mix of educational, cultural, spiritual and social activities in each participant’s local and extended communities.
